I hereby give my consent for the above named student, a fifth grade student at James E. Williams Elementary, Katy ISD, to participate in a party sponsored by the JWE PTA to be held at Hidden Lake. The party will take place on Wednesday, May 14th, 2025 from 5:00 PM to 8:00 PM. Participation in this event is strictly voluntary and parents are responsible for transportation to and from the party. In addition, my child's photo may be taken at the party and used during the end of year celebration.
It is understood that neither the Katy Independent School District, nor any of its trustees, officers, employees, or agents, nor the James E. Williams Elementary PTA, including its officers, members or volunteers, are liable for any injuries or damages done to, or caused by, the above named student at this event. I agree to indemnify and hold Katy Independent School District and the James E. WIlliams Elementary PTA harmless from all claims made against the Katy Independent School District, its trustees, officers, employees and agents, from any and all claims made by third parties which results from the above-named student being permitted to participate in this event.
I expressly waive all claims to which I may otherwise be entitled, including, but not limited to, claims for medical expenses and wages. Typing my name below constitutes my electronic signature. By signing my name below, I confirm that I have read this release and understand all of its terms and execute it voluntary and with full knowledge of its significance.
Por la presente doy mi consentimiento para el estudiante mencionado anteriormente, un estudiante de quinto grado en James E. WIlliams Elementary, Katy ISD, participará en una fiesta patrocinada por la JWE PTA que se celebrará en Hidden Lake. La fiesta tendrá lugar el miercoles 14 de mayo de 2025 de 5:00 p. m. a 8:00 p. m. La participación en este evento es estrictamente voluntaria y los padres son responsables del transporte hacia y desde la fiesta. Se entiende que ni el Distrito Escolar Independiente de Katy, ni ninguno de sus fideicomisarios, funcionarios, empleados o agentes, ni el James E.
